fix: correctly construct SingleServerInitRequest before calling handler

Fixed a TypeError caused by passing unexpected keyword arguments to the
`handle_initialize_server` coroutine. The handler expects a fully constructed
SingleServerInitRequest object, not separate keyword arguments.
